Web platform:

1. Click on observations in the menu to the left in the project view

2. Click on the observation that you want to create an action for

3. Click on yes if you want to create an action for the observation
4. Fill in the requested information
5. Click on save

6. You can add more actions by clicking on add action

Mobile application:

1. Tap on observations

2. Tap on reported by me
3. Tap on the observation you want to create an action for

4. Tap on + add

5. Fill in the information
6. Tap on add

You will now return to the detail page and can create more actions by clicking on the plus sign.
